In Jin Nyodo we know from the context which pitch to use. For honkyoku if it's written re meri that's usually an F#. But for gaikyoku re meri is F (or Kurahashi says "slightly flat of tsu").

That's really interesting that Kurahashi said slightly flat of tsu. That is what I would have thought, since it is a key transposition so as a meri note should be flatter, just like tsu meri is. So I thought to play it a little flat too (for example 75 cents up from tsu chu meri), but I was told that we should just play it the same pitch as tsu due to koto tuning issues - a kind of temperament tuning compromise from the koto tuning when shifting key I guess. But I don't have knowledge of koto in this context. I wonder what Kurahashi's koto accompaniment would play in this context? Any koto players like to chime in?
